Consumer health data we collect
What we collect depends on which service you use. Private one-on-one clients complete a full intake. Participants in our programs provide less. The categories below describe everything we may collect across all services.
- Health conditions and history. Injuries, surgeries, diagnoses, chronic conditions, chronic pain and what makes it better or worse, orthopedic injuries, allergies and sensitivities, and related medical history you share with us.
- Cardiovascular and physical readiness information. Responses to a standard physical activity readiness questionnaire, including questions about heart conditions, blood pressure, chest pain, dizziness or loss of consciousness, and whether a doctor has advised medically supervised activity only.
- Medications. Medications that may influence exercise, including their names and purpose.
- Reproductive and pregnancy information. Whether you are pregnant and, if so, how far along.
- Mental and behavioral health information. Self-reported ratings of anxiety, depression and irritability, significant personal stressors, whether you have received counseling in the past year, and any history of psychological or neurodevelopmental conditions you choose to share.
- Bodily functions and physical measurements. Mobility, range of motion, strength, balance, sleep, hydration, energy levels, and other movement assessments made during sessions or reported by you.
- Treatment and care information. Your primary care physician and other providers you identify, medical clearance letters, and precautions or contraindications identified by your provider.
- Health-related goals and activity. Exercise history, activity levels, repetitive daily movements, recreational activities, training goals, and how you respond to the movement and exercise we prescribe.
- Session records. Notes documenting what we did together, how you responded, and how your plan is being adapted.
Why we collect it and how we use it. We collect this data for one purpose: to design, deliver, and adapt safe and appropriate movement, yoga, and exercise programming for you. Specifically, we use it to assess whether our services are appropriate for you, to build and adjust your program, to document your sessions and progress, to coordinate with your healthcare provider when you ask us to, and to determine when something falls outside our scope and should be referred back to a provider.
We do not sell consumer health data. We do not use consumer health data for advertising or targeted marketing.
Sources of consumer health data
- Directly from you. Intake forms, waivers, questionnaires, email, and conversation during sessions.
- From your healthcare provider, when you have asked us to coordinate with them or they have referred you to us. This includes medical clearance letters you or your provider send us. We do not receive medical records.
- From our own observations during sessions, recorded as session notes.
Consumer health data we share
- With your healthcare provider, only with your consent, in the form of brief progress summaries or coordination about your program.
- With service providers who operate our business systems on our behalf, who may process this data incidentally in the course of storing forms, hosting files, or delivering email. These providers act on our instructions and are not permitted to use the data for their own purposes.
- When required by law, such as in response to a valid legal request.
We do not share consumer health data with advertisers, data brokers, or any third party for marketing purposes.

Your rights and how to exercise them
Under the My Health My Data Act, you have the right to:
- Confirm whether we are collecting, sharing, or selling your consumer health data, and to access that data.
- Withdraw consent to our collection and sharing of your consumer health data.
- Request deletion of your consumer health data.
To exercise any of these rights, email michelle@sagemovement.co with your request. We will respond within 45 days. If we need more time, we will tell you why and how much longer we need, up to an additional 45 days.
If we deny your request, you may appeal by replying to our response. We will respond to your appeal within 45 days and explain our reasoning. If we deny your appeal, you may contact the Washington Attorney General at www.atg.wa.gov/file-complaint.
Please note that withdrawing consent or requesting deletion may mean we can no longer safely provide services to you, because designing appropriate programming depends on knowing your health history. We may also need to retain certain records where the law requires it.
